I, a learner of Praxis Online School, understand and acknowledge that in order for Praxis Online School to facilitate this training course/module/program, and to ensure compliance with the various Schooling requirements, certain Personal Information will be required from me. 

I further understand that Praxis Online School will be required to share this information with the 3rd party that provides this training course/module/program. However, such information sharing will be limited to only what is required for compliance.

I hereby consent to my Personal Information being used for the above-mentioned purpose. 
I also consent to the class recordings, which may contain images of myself and the interaction with the teacher, being saved on the platform and used for future purposes.

However, at any point, I can request my Personal Information to be removed, redacted or rectified.
